SENSORY FUNCTION

General Observations

During interaction with the patient, note the signs of hearing deficits such as missed
communication, requests to have words repeated, reliance on lip reading, and cocking of the
head to one side in an effort to hear better. Identify eye problems by noticing if the patient uses
eyeglasses, demonstrate difficulty seeing (e.g., bumping into objects and unable to see small
print), or possess eye abnormalities such as drooping eyelids, discolored sclera, excess
tearing , discharge, and unusual movements of the eyes. Foul odors ( e.g., associated with
incontinence or vaginitis) that do not seem to bother the patient could reflect diminished
olfactory functions; cigarette burns on finger or unrecognized pressure ulcers may indicate that
the patient has reduced ability to sense pressure and pain.

As interviewing the client, we observed some signs of hearing deficits like missed
communication because the client cannot hear properly so we need to repear the questions for
multiple times. We also observed that the patient is cocking her head to one side to hear better.
The client we interviewed doesn’t use eye glasses, but we observed that she has drooping
eyelids, excess tearing and itching eyes. Other than these, we did not observed something else.

INTERVIEW

 Ask the patient about the date and type of the last ophthalmic and audiometric
examinations

ANSWER: “Naoperahan ako dati sa kanang mata, taong 2008 dahil sa cataract. Saka mga
2011 naman nung naoperahan din ako sa left eye dahil din sa cataract. Naoperahan ako noon
sa Santos Clinic sa Malolos Bulacan.”

“Sa pagkakatanda ko huling nakapag audiometric examination ako ay noong 2007 pa dahil may
dumi sa loob ng tenga ko, ang ginawa nila sakin ay may pinatak silang liquid tapos tsaka nila
tinanggal yung dumi. Sa Santos Clinic EENT sa Malolos, Bulacan.”

 Where was the examination done?

Answer: Santos Clinic, Malolos at Santos Clinic EENT

 Was an ophthalmologist or optometrist seen?

Answer: YES

 Did the eye examination include tonometry?

Answer: YES
 Was a full audiometric evaluation or basic hearing screening done?

ANSWER: NO

 If eyeglasses or hearing aids are used, ask questions about where, when, and how
these appliances were obtained (e.g., reading glasses purchased from the local
pharmacy versus prescription glasses; hearing and obtained via television
advertisement).

ANSWER: "Hindi ako gumagamit ng salamin at hearing aids.”

 Ask questions such as the following to disclose the presence of sensory problems:

 “Has there been any change in your vision? Please describe.”

ANSWER: “Laging nagluluha yung mga mata ko at tsaka parang laging may
lumulutang sa paingin ko.”

 “Are your eye glasses as useful to you as they were when you first obtained
them?”

ANSWER: “Hindi ako gumagamit ng salamin.”

 “Do you experience pain, burning, or itching in the eyes?”

ANSWER: “Madalas din kumati ang mga mata ko.”

 “Do you ever see spots floating across your eyes? How often does this happen
and how large and numerous are the spots?”

ANSWER: “Oo, laging may spots floating akong nakikita. Kulay itim ang mga ito at
parang mga lamok. Minsan pa nga ay binubugaw ko pa ng kamay ko pero wala
naman palang mga lamok.”

 “Do you ever see flashes of light or halos?”

ANSWER: “Oo, minsan.”

 “Are you eyes ever unusually dry or watery?”

ANSWER: “Madalas nagluluha ang mga mata ko.”

 “Do you have difficulty with vision at night, in dimly lit areas, or in bright areas?”

ANSWER: “Wala.”

 “Does anyone in your family have glaucoma or other eye problems?”

ANSWER: “Operada ang dalawang mata ko dahil sa karata at ang anak ko naman
ay may pugita kung tawagin sa mata.

 “Have you noticed any change in your ability to hear? Please describe.”

ANSWER: "Madalas parang may umuugong na tunog sa tenga ko.”

 “Are certain sounds more difficult for you to hear than others?”

ANSWER: “Kapag malakas ang boses ng kausap ko, mas nahihirapan akong
marinig siya.”

 “Do you ever experience pain, itching ringing or a sense of fullness in your ears?”

ANSWER: None, except of “UGONG” sounds in my ears.

 “Do you ears accumulate a lot of wax? How do you manage this?”

ANSWER: “Wala naman masyadong wax ang aking tenga at dalawang beses ako
naglilinis sa isang linggo.”

 “Is there ever drainage from your ears?”

ANSWER: “Wala.”

 “Is your sense of smell as keen as it was in earlier years? Describe any
differences.”

ANSWER: “Wala naman akong peroblema sa aking pang-amoy.”

 Do you have any problems or have you noticed changes in your ability to feel
pain, pressure, or different temperature?”

ANSWER: “Nakakaramdam pa naman din ako ng pain gaya ng pagkapaso dahil sa


pagluluto.”
