Proof of Residency: Frequently Asked Questions

1. What should I do if I submitted a proof of residency, but it was marked invalid? Proof of residency items must be
in the name of the parent/guardian as listed in Connexus®, our education management system. If your documents are in
another name, go to question two below. Otherwise, please review the following additional information, which should help
you locate a document that provides what we need:
 Utility bills: We will look to see if the service location (not the mailing address) matches the address you
provided in Connexus. In addition, the billing date should be within the standard time period that is listed on your
caretaker home page.
 Mortgage statements: The document must identify the address you provided in Connexus as the mortgaged
property. We cannot accept a document that identifies the property by account number or by a lot number. In
addition, the billing date must be within the standard time period listed on your caretaker home page.
 Lease agreements: Current lease agreements can be used as proof of residency. If your lease is very long and
you are unable to send the entire document, please be sure to send (a) the pages that identify both you and the
landlord, (b) the pages that describe the lease terms, AND (c) the signature section signed by both parties. If your
lease has month-to-month terms and was signed more than 60 days ago, you may submit it with a recent rent
receipt or a verification letter from a leasing agency on company letterhead.

2. What do I do if the acceptable proof of residency items are all in the name of someone other than the enrolling
parent/legal guardian? The person named on the proof of residency document will need to supply a copy of that
document, along with a completed, signed, and notarized Statement of Shared Domicile form.

3. What do I do it the acceptable proof of residency items are not in my name, but a Shared Domicile form is not
right for our situation? We should be able to assist. Please gather any documents you have on file that do show your
name and address and call us at 1-800-382-6010 to speak to an enrollment advisor.

What: Use this form only if your family’s proof of residency document is in the name of someone other than the parent/legal
guardian listed in Connexus®, our education management system.
Instructions: 1) Complete the parent/legal guardian section. 2) The person listed on the proof of residency document must
complete the resident owner/leaseholder section. 3) The resident owner/leaseholder must take this form, his or her photo ID, and
the proof of residency document(s) to be notarized by a notary public. 4) Submit the notarized form (one per household) along with
an acceptable proof of residency document, as described on the enrollment checklist or your Connexus home page.
