Rosario La Union, under oath, do hereby depose and say that:

1. I am the registered owner and vendor of the following parcel of lands particularly
described as follows:

A parcel of land, lot 468, Psd-1-010249 olt, being a portion of Lot 5 LRC Pcs 6627, situated at
Inabaan Norte, Rosario, La Union, containing an area of FOUR THOUSAND ONE HUNDRED
THIRTY TWO (4132) SQUARE METERS, more or less;

A parcel of land, Lot 470, Psd-1 010249 (OLT), being a portion of lot 5 LRC Pcs 6627, situated
at Inabaan Norte, Rosario, La Union, containing an area of SEVEN HUNDRED EIGHTY
FOUR (784) SQUARE METERS, more or less;

A parcel of land, Lot 551, Psd-1 010249 (OLT), being a portion of Lot 4 LRC 6627, situated at
Inabaan Norte, Rosario, La Union, containing an area of THREE HUNDRED SEVEN (307)

A parcel of land, Lot 437, Psd-1 010249 (OLT), being a portion of Lot 4 LRC PCS 6627,
situated at Inabaan Norte, Rosario, La Union, containing an area of TWO THOUSAND TWO
HUNDRED FIFTY (2250) Square meters, more or less;

A parcel land 437, Psd-1-010249(OLT), being a portion of Lot 4 LRC Pcs 6627, situated at
Inabaan Norte, Rosario, La union, containing an area of ONE THOUSAND EIGHT HUNDRED
TWO (1802) square meters, more or less;

TCT. NO. EP-819

A Parcel of land, Lot 469, PSD-1 010249 (OLT), being a portion of Lot 5 LRC pcs 6627,
situated at Inabaan Norte, Rosario, La Union, containing an area of FOUR THOUSAND FIVE
HUNDRED SIX (4,506) square meters, more or less;

I hereby attest that there is no pending case involving the aforesaid parcel of lands before the
Office of the President, before the Department of agrarian Reform and adjudication Board (DARAB), or
any adjudicator of the Department of Agrarian Reform, or before any Court or tribunal or agency
whatsoever.
2. I am executing this affidavit to attest to the truth of the foregoing facts, for all legal intents
and purposes and in support of my sale of any of the said parcel of land, as I do hereby
agree to the registration hereof with any and all agency whatsoever to give full force and
effect thereto.
