Appointment for the academic year 2020 – 2021. The said appointment will end on January 6,
2021.

I have finished my Master of Arts in Education (MAEd) last September 21, 2020 at (name and
address of school) but due to the present situation I cannot process my credentials including
my Transcript of Records (TOR) as of the moment.

With this, I promise to prove my sincerity of keeping up the education requirement for my
