1. During my site surveillance I found out that most of WELTECH personnel still not wearing
PPE’s. including some Foreman.
Potential Hazard:
1. Personnel injury fatality and property damage.
2. RFI cancellation if found by Consultant, which will lead to operation interruption,progress
delay and loss of production
Recommendation:
1. Assess your personnel and submit the required number of PPE to be withdrawn from RTCC
store which will subject for your billing deduction.( See attached photos)
