JUDICIAL AFFIDAVIT

I, Jesus B. Almojera, Filipino, of legal age, married, presently residing


and with postal address at B 12 Lot 5 Orioles Bend, Bay Breeze Executive
Village, Brgy. Hagonoy, Taguig City, after having been duly sworn to in
accordance with law, do hereby execute the foregoing judicial affidavit to form
part of my direct testimony as Petitioner in the above-titled case and hereby give
the following answers, to the question propounded by Atty. Gregorio E. Maunahan
based on my personal knowledge and authentic records.

PRELIMINARY STATEMENT

The person interrogating me is Atty. Gregorio Maunahan with law office


address at 158 Dr. Jose Rizal St., Calamba City. The interrogation is being held at
the same address on or about 1 O’clock in the afternoon of 7 August 2019. I
am answering his questions in Filipino language, fully conscious that I do so as
part of my direct testimony, under oath and may face criminal liability for
perjury, if found not telling the truth.

STATEMENT OF PURPOSE

1. To prove that I am the petitioner in the above-titled case for the Issuance
of Duplicate Original Copy of OCT No. (056) 062 covering a parcel of land
containing 7,768 square meters located at Brgy. Dila, Bay, Laguna. issued by the
Register of Deeds for Laguna, Calamba Branch on March 25, 1955; ;
2. To prove that I executed an AFFIDAVIT OF LOSS and the same was
annotated in the said title under Entry No. 2019000405 dated January 8, 2019;
3. To prove that said Title has never been encumbered or otherwise delivered
To any person or entity to guarantee any obligation and no claims has been filed
involving said title since the date of its loss;
