Dear Students,

This is a survey being carried out in the New Amsterdam Multilateral Secondary School

(NAMS) to determine whether the diet and nutrition of students in the grade eleven (11) influences their ability

to perform well or perform better. This study is being conducted as a School Based Assessment (SBA) for the

Social Studies subject. This questionnaire consists of various qualitative questions and terms involving diet and

nutrition. You are required to carefully read and answer ALL questions with honesty by indicating a tick in
