AFFIDAVIT OF LOSS OF
Original Certificate of Title No. RO-12920 (0-13273)

  I, JOCELYN S. MEGABON, of legal age, Filipino citizen, widow and resident of

Guadalupe, Bogo City, Cebu, Philippines after having been duly sworn to in accordance with
law, depose and state:

1. That I  am the surviving spouse of the late Pastor Cortes Megabon, one of the heirs of the
late Sps. Estanislao Cortes and Placida Ravilan the registered owner of a parcel of land
situated in the Poblacion, Bogo City, Cebu, Philippines and covered by Original
Certificate of Title No. RO-12920 (0-13273) and more particularly described as follows:

“A parcel of land known as Lot No. 2615; situated at Bogo City, Cebu, Philippines;
bounded on the NE: by Lot Nos. 2622, 2620 and 2616; on the SE: by Lots Nos. 2620,
2616, 3247, 3248 and 3249; on the SW: by Lots Nos. 3220, 3219, 3218, 2614 and 2612;
and on NW: by Lot No. 3217 and Road; containing an area of ONE HUNDRED
SEVEN THOUSAND SEVENTY TWO (100,772) Square Meters, more or less.”

2. That I am in possession and custodian of the said original owner’s copy title which my
late husband entrusted to me;

3. That sometime in the year 2013, during typhoon Yolanda, all my documents was
damaged and got lost including said original owner’s copy title;

4. Efforts were exerted to locate the said original owner’s copy title, but in spite of diligent
search, it could not be found and the same is now beyond recovery;

5. That the said Original Certificate of Title No. RO-12920 (0-13273) was never given, sell,
pledged, mortgaged to anybody, nor allowed anyone to use for any transaction or
disposition, there being no such transaction;

6. As such, I am executing this Affidavit of Loss to attest to the truth of the foregoing facts,
documents and the contents thereof for all its legal effects, intents, consequences and
purposes, including to request the Office of the Registry of Deeds to annotate this
affidavit of loss and for action, pending the petition with the court for the issuance of
another original owner’s of said title certificate in lieu of the one that was lost.
