Scenario Health Teaching : Hemorrhagic Stroke Case

Nurse : “good morning. Mrs.”

Client : “good morning, nurse.”

Nurse : “I would like to introduce my self, My name is Dyah , I am a nursing student at


Poltekkes Kemenkes Yogyakarta”.

Nurse : “What would you like to be called ?”

Client : “Mrs.Dyah , please.”

Family : “ And I’m Ari as a Mrs. Dyah’s daughter”

Nurse : “Ok Mrs. Dyah and Ari, I practice here from 07.00 o’clock to 14.00 o’clock. I will take
care of your mother,I hope you could cooperate so that your mother could be health again
as before. In here, I work with other health profession such as physician, physiotherapist,
dietitian. You may ask anything about treatment,particulary about something that you
don’t understand”
Client : “Thank you nurse”.

Nurse : ”I will discuss about your mother who has got a stroke. It is important to do so that after
the treatment in the hospital, you could treat your mother at home, so your mother will
not suffer from this disease again, it will take about 10 minutes, is it okay?”

Family : ”It’s okay”.

Nurse : ”Is there anything you want to ask or to do before we start ?”

Client : “Not for now”


Nurse : “Could we start right now, Ari?”

Client : “Yes, please”.

Nurse : “Mrs. Dyah, have you ever heard stroke before it? What do you know about stroke? Do
you know causes, sign and how to take care that?”
Client : “Sorry I don’t know exactly. I just know strokes attack the old people who have
hypertension”.

Nurse : “Okay then, I would like to inform you about stroke and how to take care of that.

Stroke is a condition in which a ruptured blood vessel causes bleeding inside the brain.
High blood pressure and trauma are two leading causes. Taking blood-thinning drugs
may also increase a person's risk.

The symptoms can vary based on the location of the haemorrhage in the brain, but may
include numbness or weakness of face, arm, or leg (especially on one side of the body);
confusion or change in mental status; trouble speaking or understanding speech; visual
disturbances; loss of balance, dizziness, difficulty walking; or sudden severe headache”.

Nurse : “Is there anything that you still don’t understand ?” (Is there anything that you want to
know more about this disease)

Client : “No, I have already known and understand from you what is stroke exactly, thank’s”.

Nurse : “Ok Ari, now I will explain how to care the patient”.

Patient who have stroke should have medical management for secondary prevention. We
can take care your mother arm and leg weakness can be take care with motor control
(upper and lower extremity movement) or it called range of motion (ROM). By execute
passive and active assistive ROM exercises to all extremities. It will make patients work
out a little to keep them active and prevent them from problems like joint stiffening
which might affect one’s ability to walk. Passive range of motion test must be performed
on the patients periodically so that they remain energetic and recover quickly.
You must start checking the range of motion from your mother head. Ensure that your
mother is not suffering from any injuries related to spinal cord or neck. Make her move
her head in left and right direction slowly.

After head, you must check her hand. Ask her to extend his arm towards you and then
contract it. Make her bring her arm towards his body and then move it away in a crossing
motion. Make her flex her wrist and move his fingers. These exercises must be done for
10 minutes on one arm, then next 10 minutes on the other arm. Care must be taken that
your mother does not get tired while performing these activities.

After this, you must work with her knees. Ask him to extend his knees in the forward
direction and then contract them to bring close to the center of the body. This activity
must be performed for 10 minutes on one leg.

Afterwards, ask the patient to stretch at the ankles and twist and turn the toes in
clockwise and anti-clockwise directions.

Then, could you please practice the step to do ROM correctly?”

Family : “Yes sure.” ( The client practice how to do ROM to the patient)

Nurse : “That great Ari. I hope you can practice the ROM to your mom correctly”.

Client : “Ok, absolutely”.

Nurse : “Ari could you explain what we have already discussed, please?”

Family : “Yes of course. Stroke is a condition in which a ruptured blood vessel causes bleeding
inside the brain causes by hypertension. The symptoms include numbness or weakness of
face, arm, or leg (especially on one side of the body), so I can take care my mother with
the range of motion (ROM) to exercises to all extremities. Is it correct, nurse?”

Nurse : “Yes Ari, you’re great. From the discussion I can make a conclusion that you have
already known about stoke and how to take care your mother at home”.

Family : “Yes, that’s true. Thank you, nurse”.

Nurse : “You were so active while we were discussing about this disease”.
Family : “My pleasure”.

Nurse : “Ari I will write the result of this discussion on your mother’s status. Thanks for your
attention, see you”.

Family : “You’re welcome. See you”.
