ROLEPLAY DIALOGUE

On October 28, 2016 at 11.30 WIB, the patient came to the William Booth
Hospital emergency room with an injury to his lower extremity due to a car
accident that happened to him. When studied, the client complained of pain in the
left leg, the client complained of pain when doing a little movement /
mobilization. After the MRS patient for 3 days the patient looked anxious, scared,
worried and tended to just immobilize.

(after 3 days of MRS)

REASSESING THE CLIENT

Nurse Indri : good morning ma'am? 

Patient : Good morning sus                                         

Nurse Indri : My name is nurse Indri, I am on duty this morning from 06.00 to
14.00 WIB who will be responsible for maternal care. Can you
state your full name and date of birth?             

Patient : Oh yes, my name is Yeriko Eky, I was born on December 27,


1969             

Nurse Indri : Excuse me, mom, I'll check your patient's bracelet first. How do
you feel this morning? (while checking the patient's TTV)              

Patient : Good sus                                         

Nurse Indri : Is your mother's leg still sore / sore? Are you able to move your
feet a little bit?             
Patient : It doesn't hurt too much but I still have trouble moving my legs
everywhere, it's hard to change positions              

Nurse Indri : I see ma'am, yes-yes I understand ma'am                           

Patient : I also feel anxious, afraid, and worried about breast                                          

Nurse Indri : What happened?                            

Patient : I am afraid I can't do anything else, my leg is like this forever,


when will it heal, yes? Is my leg too bad to heal? And when can I
go home and do my usual activities?              

Nurse Indri : I see, ma'am. yes ma'am, calm down, ma'am, don't worry.
Because the mother said that she was worried, afraid, worried
about when the leg pain healed. Mother asked when she would
return home, what if we just talked about mother's anxiety and I
will explain it to you.                

Patient : Yes, I want it                                     

Nurse Indri : How long do you want to talk about this? How about 15 minutes
ma'am?               

Patient : Yes, okay                                         

Nurse Indri : Where do you want us to talk about this? Is it in this mother's
room or where?   

Patient : Yes, just in my room             

Nurse Indri : Excuse me mom, in an hour I'll be here again to run the contract
that we agreed on earlier   

Patient : Yes, thank you             

Nurse : You're welcome

 DETERMINING THE NURSES NEED FOR ASSISTANCE


Nurses Indri and Iza : Excuse me mom            

Patient : Yes, sus                                     

Nurse Indri : Oh yes ma'am, in connection with the contract we agreed on


earlier, I will carry out treatment and educate you about fracture
recovery so that you don't worry anymore and help you to do light
mobilization, ma'am.                         

Nurse Indri : Yes, Ma'am, there is also Sister Iza who will replace me to teach
mothers light mobilization and also education about the phase of
fracture healing. How mother is you willing?                        

Patient : Yes, it's okay                                     

Nurse Indri : Then I'm sorry ma'am I'll excuse you first, you can continue with
Nurse Iza                        

Patient : Yes, thank you                                     

 IMPLEMENTING THE NURSING INTERVENTION

Nurses Iza : Well my mother would teach mothers lightweight mobilization


techniques so that mothers do not lie on your back only. First,
mom, if you feel tired on your back and here there is a mother's
family who can keep the mattress up a little, the head is up to 65 °,
and you can also tilt it like this starting with tilts slowly ma'am, it
feels heavy later on the legs but if mother trying a little mother can
change positions

little

Nurse Iza : Now I will explain the phase of fracture healing, ma'am, first the
fracture healing is a period of inflammation before the wound will
make the surrounding tissue inflamed which is marked by swelling,
redness and a warm feeling and of course, it hurts. This stage starts
on the day the fracture occurs and lasts about 2 to 3 weeks. Next
stage 2: Formation of soft callus (soft callus) Between 2 and 3 after
injury, pain and swelling will begin to disappear. In the fracture
healing stage, smooth callus is formed. This stage usually lasts up
to 4 to 8 weeks after the injury. Stage 3: Formation of a hard callus
(hard callus) Between 4 to 8 weeks, with a period of 8 to 12 weeks
after injury, new bone has filled in the fracture. Stage 4: Bone
remodeling. Beginning approximately 8 to 12 weeks after injury,
the fracture side undergoes remodeling (repairing or remodeling)
repairing any defects that may remain as a result of the injury. This
is the final stage of fracture healing, which can last several years.
Nurse : how do you understand? Or is there something less
clear?        
            

Patient : Yes, it's clear, I have calmed down a little now               

Nurse Iza : How can you mobilize lightly?      

 (then the patient repeats what mobilization action the nurse said)

Patient : (While watching the patient) Yes, I can but a little I can change
