This is for the information of all the employees and students of the Institute that Routine Immunization
programme for all the eligible children (having Valid Medical Booklet) is going to start w.e.f. 12.12.2017
(Tuesday) at the Institute Hospital.
The details of Vaccination are given below :
1. A vaccination card will be provided to each child, eligible for vaccination.
2. IMMUNISATION will be done ONLY for Vaccines covered under Universal Immunisation
Programme (UIP) of Govt. of India. Request for any vaccine not covered in UIP may not be
possible to entertained.
3. Registration of Children for Vaccination will be done on every Tuesday during morning OPD
hours, in the Emergency Room of the hospital. Parents should report for the Registration with the
following :
(a) Valid Medical Booklet of the child to be vaccinated.
(b) Prescription of the treating Pediatrician, clearly mentioning the
(i) name of Vaccine (ii) Due date of Vaccine
4. Vaccination of all the registered children will be done on every Thursday from 9:00 AM to
11:00 AM in Emergency Room.
