AFFIDAVIT OF ATTACHMENT OF SIDECAR

I, MARVIN B. SILVANO, of legal age, married, Filipino and with
residence address at #280 Balas, Mexico, Pampanga, after having been duly
sworn to in accordance with law, hereby depose and state:

1. That I bought a motorcycle from Ronnie L. Francisco by virtue of Deed of

Absolute Sale. As evidence, attached herewith is the copy of Deed of
Absolute Sale marked as Annex A;

2. That said motorcycle is more particularly described as follows;

motorcycle to motorcycle with sidecar and attest to the truth of the
foregoing and in support of my request to change the entry in the body
type section to motorcycle with sidecar in the motorcycle’s OR/CR.
