TAKE AN AXILLARY TEMPERATURE DEFINITION Measuring body temperature use axillary thermometer which put in middle of armpit. EQUIPMENTS - Axillary thermometer - Small towel or tissue (for dry the thermometer) - Bowl contain soap water - Bowl contain clean water - Bowl contain desinfectan liquid - Waste container - Pencil or bolpoint PREPARATIONS OF PATIENT Explain to patient about the purpose and the procedure PREPARATIONS OF NURSE Wash hands PROCEDURE 1. Drawn near equipment to patient 2. Arrange position of patient sit or supinasi 3. Open the clothes from shoulder until up arm 4. Dry patient’s armpit use small towel 5. If thermometer put in desinfectan liquid, before using input thermometer into bowl contain clean water, then dry using tissue from middle to tip of thermometer as twirled 6. Read the height of mercury in thermometer. If upper 35oC, degraded mercury by hold on tip of thermometer strongly and keep away from hard object. Then shaky quickly from up to down until the height of mercury under 35oC. 7. Set thermometer until reservoir part in middle of armpit 8. Assure thermometer touched surface of skin 9. Crosswise patient’s hand to chest 10. Wait 5-10 minutes (for children 5 minutes and for adult 10 minutes) 11. Take thermometer and dry thermometer from middle to tip of thermometer as twirled 12. Read the result of measurement a line with nurse’s eyes 13. Clean thermometer use soap water, then put into clean water after that dry using tissue 14. Degrade the height of mercury in thermometer until scale under 35oC and return in container or put into desinfectan liquid 15. Inform the result of measurement to patient 16. Neaten patient’s clothes 17. Neaten the equipment and environment 18. Wash hands 19. Evaluation respond the patient 20. Documentation the result by make graphic or curve in page of medical record correctly.
