Honorable Court, submit their formal offer of exhibits, thus:

Alvañiz; petitioner Donna Marrie
Alvañiz was born on
August 4, 1978;

“F-2” The Birth Certificate of the petitioner Ken Offered to prove that
Anthony; petitioner Ken Anthony
was born on October 30,

“G-1” Marriage Contract of petitioner’s parents Offered to prove that


petitioners parents was
married on September 10,
1974;

“H” Death Certificate of Reynaldo Holiva; Offered to prove that


petitioners’ father died on
October 31, 1995;

“I” Marriage Contract of Reynaldo Holiva and Offered to prove that

married to a certain Leticia
De Guzman on December
15, 1968. This marriage was
not dissolved in any
manner whatsoever before
or even after Reynaldo
Holiva married petitioners’
father on September 10,
1974;

“J” Court Order of RTC Branch 11, dated June Offered to prove that
27, 1990; petitioners’ mother filed a
case in court for the
declaration of nullity of her
marriage to Reynaldo
Holiva wherein in the
same case, Civil Case No.
CEB-8782, Reynaldo
Holiva admitted that his
marriage to Letecia De
Guzman was still
subsisting at the time of
the celebration of the
second marriage, and
therefore the court ruled
that it is definite that the
second marriage was void
ab initio.

“K” Entry of Finality dated June 5, 2009; Offered to prove that in

Reynaldo Holiva and
petitioners’ mother entered
a compromise agreement
dated November 16, 1990
and the same was
approved in an Order
dated November 26, 1990.
Said Order dated

Finality dated June 5, 2009;

“L” Baptismal Certificate of Zoe Kristoffer Offered to prove that


petitioner Zoe Kristoffer
Alvañis is known in the
community as Zoe
Kristoffer Alvañiz even
though in his Certificate of
Live Birth his name
appears as Kristoffer
Alvañiz Holiva. To avoid
confusion, Zoe Kristoffer
Alvañiz prays to this
Honorable Court that he be
allowed to change his
name to Zoe Kristoffer
Alvañiz;

“M” NBI Clearance of Zoe Kristoffer Offered to prove that

has none of the
disqualification for his to
change his name;

Exhibits “Q”, “L”, “R” and “P” were already submitted to the court
during the September 8, 2010 hearing while the rest of the exhibits are hereto
attached.

WHEREFORE, in view of the foregoing, petitioners pray to this Honorable


Court that the foregoing exhibits be dully admitted;
