Instruction:
1. This form will be distributed by respective coordinators during the last paper of the final term exam. Coordinators
will also announce final submission date of this form.
2. Students will fill it and get it signed as required immediately after the announcement of results. Then the students will
submit the form to the college main office on the due date along with fee deposit receipt.
3. In the main office student record will be updated and list of students (attendance sheet) will be prepared for the new
semester.
Note: Semester rules shall apply. Moreover, the student will deposit the fee if application is approved.
