THAT, WE, BIBIANA ALFONSO and NATALIA G. TORRENDON, both of legal
age, Filipino Citizens and residents of La Castellana, Negros Occidental, Philippines,
after having been duly sworn to an oath in accordance with law do hereby depose and

That we personally know SPOUSES RACEL C. GREGORIO and CORAZON C.
CELESTE, both Filipino citizens and residents of La Castellana, Negros Occidental

That SPOUSES RACEL C. GREGORIO and CORAZON C. CELESTE were

Merried before HON. TRANQUILINO N. CHUANICO, Municipal Mayor of La Castellance,
Negros Occidental on March 31, 1996;

That on the Certification of Marriage Registry No. 96- 136 issued by the Civil
Registrar of La Castellana, Negros Occidental the date of birth of the husband RACEL
C. GREGORIO was erroneously entered MARCH 6, 1969 instead of February 23, 1969
the true and correct date of birth of the husband as appearing in the Certificate of Live
Birth issued by NSO;

That RACEL C. GREGORIO born on March 6, 1969 as erroneously entered in

the Certificate of Marriage and RACEL C. GREGORIO who was born on February 23,
1969 as entered in the Certificate of Live Birth issued by NSO ARE ONE THE
SAME PERSON;

That we come to know of the above mentioned fact because we are neighbors
and friends of the above mentioned family;

That we are not related to the subject person either by consanguinity or by

Affinity and that we are not interested in the matter whether actual or contingent;

That we are executing this affidavit voluntarily in order to establish the fact that
The true and correct date of birth RACEL C. GREGORIO the husbond of CORAZON C.
CELESTE should be FEBRUARY 23, 1969 as entered in the Certificate of Birth
Issued bs NSO and not MARCH 6, 1969 as erroneously entered in the certificate of
Marriage issued by the Civil Registrar of La Castellana, Negros Occidental but the two
Refers to ONE AND THE SAME PERSON.
