MOTION TO RELEASE MONETARY AWARD

COMES NOW the complainant, pro se, and unto thisHonorable Office, respectfully avers that:

1. By virtue of the Decision by NLRC dated 09 July 2018 and Certificate of Finality dated 23
November 2018, complainant moved for the issuance of a writ of execution on 11 December
2018.

2. In an Order dated 13 December, 2018, the Honorable LaborArbiter granted the motion and issued
a Writ of Execution in favor of the complainant.

PRAYER

WHEREFORE , in view of the foregoing, it is most respectfully prayed unto this Honorable Office to
order the Union Bank of the Philippines to accordingly cause the release in favor of the complainant the
adjudged monetary award.
