Bani, Pangasinan , do hereby name, constitute, and appoint BRYAN ORTALEZA SADAKEN,
to be my true and lawful attorney-in-fact, for me and in my name, place, and stead, to do and
perform the following acts and things to wit:

To request, demand, receive, and acknowledge receipt any and all money in the form of
backpay and other amounts due me from the Human Resource Department of SM Southmall-
Las Piñas.

To make, sign, execute and deliver documents proof of identification and other writings
necessary to accomplish the acts aforementioned.

HEREBY GIVING AND GRANTING unto my said attorney-in-fact, full power and
authority whatsoever requisite or necessary or proper to be done in and about the premises as
