I, JANE RAMIREZ, Filipino, single of legal age, with address at 375 Lallana Street, Barangay 93,
Zone 8, District 1, Tondo, Manila, after being duly sworn according to law, hereby depose and state
that:
1. I personally know “Edmund Joseph Ramirez” as my father and have used the same in his personal
and official records and interactions, as shown by the following attached identification documents, refer
to one and the same person.

2. I also know for a fact that the above person and the person by the name of “Edmund Josef Ramirez”
as appearing in the attached identification documents, refer to one and the same person;
