AFFIDAVIT OF RESIDENCY

I, JAMEL M. AGANTAL, Filipino, of legal age, single, residing at Purok 10, Bago
Oshiro, Tugbok District, Davao City, after having been duly sworn in accordance with
law, hereby depose and say that:

1. I am the registered owner of a Brilliant Silver Nissan Urvan (“subject car” for
brevity) with plate number LAB 9341;

2. On the eve of August 18, 2020, I parked the subject car within the premises of
my house and went to sleep;

3. The next morning, I was surprised to see that the right side of the subject car,
particularly on the outer portion of the front passenger side has dents and
scratches;

4. Upon further investigation, the backlight of the subject car, also on the right
side, was damaged;

5. Despite due diligence and taking precaution, I could not have prevented the
same;

6. I am executing this affidavit to attest to the truth of the foregoing and to use
the same for whatever legal purpose it may serve.
