I, GERLIE MAE C. SABIDO, Filipino of legal age, married, and
a resident of Purok Upper Valley, Brgy. Sto. Niño, City of Koronadal, depose and say:
1. That I am a duly enrolled second year Juris Doctor student
of the Notre Dame of Marbel University (NDMU) College of Law for the Academic Year 2020-2021;
2. That I am aware and voluntarily conformed with the Letter
Request for a face to face midterm examination on October 26 – 31, 2020;
3. That I am fully informed of the current health status of my
classmates relevant to Covid-19 exposures as well as the risks this may pose to me during the face to face midterm examination;
4. That I do hereby promise to undertake all health protocols
and precautionary measures, personally and within my immediate environment for the entire duration of the midterm examination, such as but not limited to: wear face masks and face shields at all times; sanitize regularly within the day; keep a safe distance (social distancing) at all times, wherever I maybe; file a leave of absence; stay in the safe zones only (e.g. home – school – workplace); lessen if not avoid interactions with other people; no travel outside the City of Koronadal and other hot spots where there are increased number of Covid-19 transmissions.
5. That I am personally accountable for this undertaking and
will not hold responsible the NDMU College of Law and the NDMU Administration as a whole for exposure to Covid-19 infection should there be any.
