“Almost Fainted On Getting Up” Oo

63-year-old man presented to hospital with attacks of dizziness and dimness
of vision on standing up, occurring particularly after meals since one month
ago. During some such attacks, he almost fainted.

No history of drug therapy or fluid losses was given.
His blood pressure was 100/70 and 80/55 mmHg in the recumbent and upright
postures, respectively. His pulse was 86 beats/min, regular. Blood sugar and
other tests revealed that the patient was diabetic. After few other tests, the
attending physician told the patient that he may have been diabetic for many
