For Wedding Services

I, ____________________________________________ residing at Brgy.

Buenaswerte, Palanas, Masbate. I RENTED ALL THE GOWNS in my wedding entourage
included make-up services and reception decoration at CYPRES GOWN RENTAL &
SALON, Nursery St., Masbate City. Our agreement is this: the transportation for all the
utensils using in the reception and the other clothing materials, and props arts for the
decorations, the transportation provide by the groom & bride which is car/truck for
loading. I agree to pick-up and returned them after the occasion in vice versa services.
Point to point where is their place belong. All the workers and personnel of Cypres will
be included to ride in the said car/truck service; and to be sure all them are safe &
good condition, after in the said occasion, and with free foods and accommodation.

 Place of wedding : Brgy. Buenaswerte, Palanas, Masbate.

 Date of Wedding : January 18, 2020
 Date of Pick-up : January 16, 2020
 Date of Return : January 18, 2020 PM.
