We, the Grade 12 STEM students of St. Rita’s College of Balingasag, are conducting a research
entitled, “Students’ Perception on the Influence of Classical Music on their Cognitive
Performance” as part of the requirements for Practical Research I.

In this regard, we are asking for your precious time and effort to allow us to conduct a
virtual interview with you, as this will serve as a vital component for the completion of
the study. The interview would only last for about 10 to 15 minutes and would be
arranged at a time convenient to your schedule. Participation in the session is entirely
voluntary. The records collected from this study will be kept in the highest level of
confidentiality. No individual identities will be used in any reports or publications resulting from
the interrogation. All interviews will be given codes and stored separately from any names or
other direct identification of participants. Audio recordings will be destroyed once they are
analyzed and transcribed.

To the best of the researcher’s knowledge, there will be no more risk of harm than you would
normally experience in your daily life. The anticipated risks associated with participation in this
research will be minimal.

We believed that you are with us in our enthusiasm to finish this requirement as compliance on
