AFFIDAVIT OF SUPPORT AND CONSENT

I, ALLAN C. SALVACION, of legal age, married, Filipino citizen, with residence and postal address
at Blk-7 Lot-29 Lovebirds St., Camella Woodhills, Brgy. San Antonio, San Pedro, Laguna, after having
sworn to law depose and say:

That I am a regular employee of the Commission on Audit (COA) presently assigned at COA
Regional Office No. IV-B, Quezon City and the father of my daughter whose name is LIZA CELICA M.
SALVACION, a Licensed/Registered Nurse, presently employed at Lipa City District Hospital, Lipa City,
Batangas;

That being the father of my daughter, LIZA CELICA M. SALVACION, I supported and spent her
pre, elementary, high school and college education until she graduated and become a
Licensed/Registered Nurse and eventually employed at the said Lipa City District Hospital;

That I hereby give my consent for her to study at the Institute of Health & Nursing Australia
(IHNA), Melbourne, Australia, and will continue to support and shoulder all her expenses for that
purpose;

That I executed this affidavit to establish the veracity of the foregoing statement.
