Subject ： Engine #1, break down.

1. Summary
Observe abnormal noise from engine right bank cylinder #13 at 11:30 AM 01/01/2009.

Stopped down for inspection.

2. Finding/Damage
After opening cylinder head cover find Brocken Outher spring on exhaust valve.

Inner spring was still OK

3. Cause of Damage
Checked valve clearances, and was in normal range 1mm-EX & 0.5mm-IN

Checked lubrication of valve and rocker arm mechanism- was Ok

Damage probably caused by defective spring itself.

4. Action plan / Countermeasure

Removed cylinder head and start clean all parts, prepare for installation

Request all necessary spares from town. On 2/01/2009

Receive parts on 3/01/2009. and start installation immediately.

09:00 AM 04/01/2009 test and put under load 1000 KVA

5. Remarks
Installed same cylinder head, with new spring.

Received new remanufactured cylinder head kept in warehouse!
