Lesson Week Formative Assessments

Math

Learning Objective:​ ​By designing a rainbow chart and answering given questions, students will

demonstrate ways to make 10, with 75% accuracy.

Description of Assessment:​ The students will be asked to hold their work up to the camera and

the teacher will use a checklist to determine whether or not the student was able to answer the

question correctly.

Rationale:​ This assessment is aligned with the learning objective because it allows the teacher to

see if the students are able to correctly show the ways to make ten, meeting the objective of 80%

accuracy, or six out of eight correct.

Scoring Guide:
No differentiated scoring guide was incorporated in this assessment. Because the student who is

on an IEP had a hard copy of the content being taught, and all students were given instruction

and examples, all students were graded by the checklist.


Science

Learning Objective:​ While completing the verbal question activity living vs. nonliving things

activity, the students will show understanding of what living things need to survive, with at least

5 out of the 6 questions correct.

Description of Assessment:​ The teacher will use a checklist to determine whether the students

were able to answer five out of six questions correctly.

Rationale:​ This assessment aligns with the learning objective because the questions test the

students ability to identify the needs of living things and the checklist helps the teacher to record

this information.

Scoring Guide
There is no differentiated scoring guide for this assessment as it is verbal questions that the

students will listen to. If the students were at a higher level and had reading abilities, a written

form could also be provided; however these students are not yet fluent readers.
English Language Arts

Learning Objective:​ By using a read aloud text and drawing activity, students will work to

identify the major events, including the rules, with no more than two attempts.

Description of Assessment:​ The teacher will ask the students to draw a picture of the character

following a rule that was discussed. The teacher will then use a checklist to determine if the

student was able to do so with no more than two attempts.

Rationale:​ This assessment aligns with the learning objective because it sees if the student is

able to identify the major events

Scoring Guide
There is no differentiated scoring guide for this assessment as it is focused on the student’s

ability to show or explain what happened in the story through a drawing. The drawing itself is

not being graded, but rather the student’s ability to use details from the story to do so.
​Social Studies

Learning Objective:​ By answering questions on a Google Form, the students will correctly

identify rules and fairness with at least 75% accuracy (3 out of 4).

Description of Assessment:​ The students will be asked questions related to rules, fairness, and

consequences on a Google Form.

Rationale:​ This assessment aligns with the learning objective because it gets the students to

identify their knowledge and allows the teacher to easily see their ability to meet the objective.
