Dear Mayor Tom:

Greetings of peace in this yuletide season.

I, Elena T. Yuta, is a resident of Binanuaanan Sur Pili, Camarines Sur, a widow and a
member of the Senior Citizen of this municipality. On November 17, 2020, I was admitted in the hospital
for hypertension and other related ailment that confined me until to date. The last laboratory results as
required by my physicians warranted me to be discharged from the hospital and just continue my
medication at home.

Hence, in connection with this, I would like to request some medical financial assistance
in order to defray the cost of my hospital bills. Attached herewith is the certification coming from the
barangay that I am an indigent resident having no source of personal income.

I’m hoping that you would find ways to grant this humble request.
