AFFIDAVIT OF ACKNOWLEDGMENT OF PATERNITY

I, JERBYS COLE N. LIKIYAN, Filipino citizen, of legal age, and a resident of

Barangay Rosario, Santiago City, after having been duly sworn to in accordance with
law, do hereby depose and say:

1. That I am the biological father of the child JOHN SYREX JALLIGAO

GONZALES who was born on March 30, 2010 in Southern Isabela General
Hospital, Rosario, Santiago City, Isabela;

2. That at the time of the birth of said child JOHN SYREX JALLIGAO
GONZALES, I was not legally married to her biological mother, JUNARISA
JALLIGAO GONZALES;

3. That I hereby acknowledge my paternity/filiation of the child JOHN SYREX

JALLIGAO GONZALES;

4. That, further, I am giving consent for the child to use my surname and that the
same be reflected in his birth certificate;

5. That I am executing this Affidavit to attest to the truth of the foregoing facts
and for whatever legal purpose it may serve.
