AFFIDAVIT OF DISCREPANCY

I, APRIL JOY R. GAYAP, of legal age, single, Filipino, and a resident of

Purok-5 Sagana, Santiago City, after having sworn to in accordance with law,
hereby depose and state that:

1. I was born on April 25, 1998 in Purok-5, Sagana, Santiago City to

Marlyn Rombaoa and Danilo Gayap;

2. I was baptized on April 25, 2016 according to the rites and

ceremonies of Espiritu Santo Cristiano Alpha Omega ABBA AMA
Christ in Santiago City, Isabela with the name APRIL JOY R.
GAYAP, as appearing on my Baptismal Certificate. A copy of my
Baptismal Certificate is hereto attached as ANNEX “A”;

3. Since childhood, I am known by my name April Joy R. Gayap

which I have been using in all my personal records, transactions
and communications. A copy of my High School Diploma and
Elementary Diploma are hereto attached as ANNEXES “B” and
“C”, respectively;

4. The name appearing on my Certificate of Live Birth registered

with the Civil Registry of Santiago City and the Philippine Statistics
Authority is APRIL JOY ROMBAOA. A copy of my Certificate of
Live Birth issued by the Philippine Statistics Authority is hereto
attached as Annex “D”;

5. The surname, as appearing in my Certificate of Live Birth is that of

my mother. The reason being is that when I was born, my parents
were yet to be married. Eventually, they were married on March
26, 2001. A copy of their Certificate of Marriage is hereto attached
as ANNEX “E”; and

6. This affidavit is further executed to attest to the truth of the

foregoing facts and for all intents this might be of legal worth;
