a. Original birth certificate of the child, alongwith one copy dully attested copy by a JP.
b. JP attested copies of both parent’s Pakistani passports and National Identity Cards.
c. 03 photographs of the child (child’s name written on the back).
d. JP attested copy of the parent’s marriage certificate.
e. Letter of consent from a parent (if one of them is a foreigner).
NOTE.
a. Application form for registration of birth should be submitted within 6 months of the birth of the child,
along with the following documents.
b. For prompt service, please ensure that the above requirements are completed. Failure to observe the
above may result in delay for which High Commission will not accept responsibility.
