INVESTIGATION REPORT

The investigation was conducted by Mr. Jerson Daligdig (Graids Safety Officer) @ 03:50 PM
on November 20-21, 2020 in relation to the incident report made by CRO L. Magsayo dated Nov.
15, 2020 on issue of leaving area in HA Preparer and Bagging Sewer with no incoming shift reliever
respectively.

Question and Answer:

Q1. Mr. Ferrer (Micro Preparer), why did you leave your area without the proper reliever of your
post?

On November 15, 2020 (Sunday) my shift was 8-4. Since it will be changed shift on the
following day wherein I have to report back on 12-8 shift, had asked approval of my Foreman that I
will go home and he himself shall make the follow to Mr. Largo who would then be my assigned
reliever. Mr. Largo (Micro Assist) was late for few minutes as he claimed he’s bit tired working 12
hours for straight seven (7) days due to suspension of their co-worker on Micro Assist.

Q2. Why no Bagging Sewer as claimed by CRO during 4-12 shift?

Per investigation conducted the regular 4-12 sewer Mr. Itom was still under suspension and
was replaced by Mr. Mirantes (from intake) who arrived earlier before the 4-12 shift start.

Conclusion:

There could had been misinformation to the CRO who are those relievers to take over for
those were suspended under his shift. The 4-12 shift on this particular incident report run
smoothly with no shortage of manpower whatsoever.

Moving Forward:
To avoid recurrence of this incident, Graids management will conduct re-orientation of all
workers to remind again their core responsibilities of the task assign to them. Re-orientation also
includes establish Code of Conduct so they will be reminded on the penalties if ever they deviate
from the rules. Leaving post when not properly relieved and AWOL will be highlighted on this re-
orientation as these were the common offenses had been on record.
