CARE OF THE GROWING CLIENT


CASE ANALYSIS

Instructions: Each members of the group are assigned to a particular task. Follow the
given task guided by the case scenario. Make a comprehensive output.

GROUP 1: Infants
Case scenario: A 9-month-old infant is admitted due to high grade fever of 39 ° C, four
episodes of vomiting and five episodes of loose bowel movement. Upon assessment,
sunken eyes are noted, lips are cracked and pale, respiration is 50 breaths/min, pulse of
120 beats per minute. Bowel sound is 35 cycles heard on the right lower abdomen. The
client has lost appetite since admission.

The physician ordered D5IMB 1L at 60cc/ hour to be infused at the left arm. Other drugs
were also ordered to be given specifically Paracetamol (Tempra) 120mg/ml 2ml PO every
4 hours

GROUP 2: Toddlers
Case scenario: A 3-year-old toddler was admitted due to fever and productive cough.
Upon assessment, fast breathing was noted. A crackling sound was heard upon
auscultation. Oxygen via cannula was attached at 2 lpm. Her vital signs revealed the
following: Temperate: 38.5 °C; Pulse: 100 bmp, Respiration: 60 breaths/min

The client has a loss of appetite, difficulty sleeping at night due to persistent cough and
appears weak and tired. The physician ordered D5IMB 1L at 60cc/hour to be infused at
the left arm. Other drugs ordered to be given. One drug ordered is for the cough.
Salbutamol Syrup 2mg/5ml 2.5ml PO TID

GROUP 3: Preschoolers
Case scenario: A 4-year-old preschool child was admitted due to falls. The client had an
accident while climbing a tree with his friends. He sustained a fracture on his left arm. The
client underwent an operation to correct the fracture two days ago. Upon assessment, the
client verbalized pain to the affected body part with a rating scale of 6/10. He was seen
crying and appeared nervous whenever a nurse approached him. He does not want to be
separated from his mother. Vital signs revealed: temperature: 37.0 °C; pulse: 90 bpm,
respiration: 30 breaths/min.

The physician ordered IV fluids to be infused. Acetaminophen 50mg IVTT OD was given
for the pain.

GROUP 4: Adolescent
Case scenario: A 15-year-old client was admitted due to loss of appetite. She has lost
her appetite for months and has not been eating well. She verbalized that she is trying to
go on a diet since she does not want to gain weight and disturbed her body image. She
has lost weight from 50kgs to 30 kgs in two months. She has also difficulty falling asleep
at night due to activities related to school and friends. Her laboratory results revealed
electrolyte deficiency and anemia.

The physician ordered IV fluids and drugs to regain her condition. Vitamins B complex
500mg 1 tablet PO OD was ordered to stimulate her appetite.

GROUP 5: Young Adult
Case scenario: A 28-year-old primigravida woman just gave birth to a healthy baby girl.
Upon assessment, breasts are enlarged, dark areola and protruded nipples. Upon
palpation, fundus was located 1cm below the umbilicus. Linea nigra and striae gravidarum
were noted on her abdomen. Presence of episiotomy wounds and lochia rubra were also
noted. The client was able to void 6 hours after delivery but was not yet able to defecate.
Oxytocin 10 units was infused on her IV fluids to prevent bleeding.

Case scenario: A 40-year-old adult was admitted due to severe anxiety. She seems
restless, nervous and has trouble concentrating. The client verbalizes a feeling of panic
whenever she is faced with problems at home and at work. She suffered this condition
for years and only sought consultation when she had an attack and hit her head hard after
being unconscious. The physician ordered alprazolam (Xanax) 0.5 mg 1 tablet PO BID
to help relieve her anxiety.
