AFFIDAVIT OF LOT OWNER’S CONSENT

I, ----------, of legal age, Filipino, single, and a resident of Alta Tierra, Tiguma, Pagadian
City, after having been duly sworn in accordance with the law, hereby depose and state:
1. That I am the registered owner of lot 2-----C-1, PSD-09-041480 (Pls-119), located
at Brgy. Poblacion, Pagadian City, Zamboanga del Sur, containing an area of
Ninety-Four (94) square meters, covered by Certificate of Title No. 13------------.

2. That my Mother, Grace C. , of legal age, Filipino, married, and a resident of Alta
Tierra, Tiguma, Pagadian City is the Owner of GRAZZI---------;

3. That I am willing to allocate the aforementioned parcel of land for the construction of
a building and/or establishment of a business to my Mother;

4. That by these presents, I give my CONSENT and AUTHORIZED Gngay, to use lot
2---1-C-1, PSD-09-041480 (Pls-119), for the establishment of her Business “GRA”;

5. That I am executing this affidavit to declare the truth of the foregoing facts and for
whatever legal purpose it may serve.
