AFFIDAVIT OF LOSS & DISCREPANCY

I, CARMELITA D. CORPUZ, Filipino, of legal age, married, and a resident of Lot 10 Blk 3, St.
Luke St., Villa Maria Subdivision, Barangay 168, Deparo, Caloocan, after having been duly sworn in
accordance with law, hereby depose and say:
1. That I am the owner of a Postal I.D. issued by the Philippine Postal Corporation bearing
the name “Carmelita D. Corpuz”;
2. That in the above-mentioned Postal I.D., it appeared that my birthdate was February 3,
1972;
3. That in my Certificate of Live Birth issued by the Philippine Statistics Authority, my TRUE
and CORRECT birthdate is February 3, 1971;
4. That I lost said Postal I.D. last May 2019 and that I exerted all efforts to locate the same
but I could no longer find it and it is considered lost for all legal purposes;
5. That said Postal I.D. was not confiscated by any officer of the law by reason of any
violation of any law, ordinance, presidential decree and neither was it offered as collateral
for any loan;

6. That I am executing this affidavit to attest to the truth of the foregoing facts and to reconcile
and correct the apparent discrepancy of my birthdate in my LOST Postal Identification
Card (PID) as well as to support my request for reissuance of a new Postal I.D. and for
whatever legal purpose it may serve.
