How to upload your completed form and certificates to the UCL Applicant Portal
Please upload the completed form and certificates/transcripts as one file to the UCL Applicant Portal
by the deadline stated in the accompanying email. Documents must be in one of the following
formats: PDF, JPG, DOC and DOCX and less than 2MB in size. Please make sure ALL files uploaded
are named appropriately e.g. “GCSE English.pdf”.
The UCL Applicant Portal (www.ucl.ac.uk/portico) is an online service which will allow you to track the
progress of your application. You should have already been sent details of how to log in from the
email account donotreplyadmissions@ucl.ac.uk, so please check it has not entered your junk folder.

Academic Qualifications
Final first degree certificate/transcript, or your results to date (if you are yet to complete the
qualification). If you have not undertaken a degree please upload the certificate for your
highest level qualification

Fitness to Teach
Teacher Training providers have a responsibility to ensure that trainees have the health and physical
capacity to teach and will not put children and young people at risk of harm. For this reason all
applicants who accept an offer of admissions to UCL will be required to complete Occupational Health
and Disclosure Barring Service (DBS) Enhanced Disclosure checks. In addition to this, if you have
lived, worked, studied or travelled in a country outside of the UK for three months or more, in the last
ten years, you will be required to obtain a Certificate of Good Conduct from that country. To help us
determine whether you will require this additional certificate please complete the following table with
your address history over the last TEN years (adding rows as appropriate).
If you are going to be spending 3 months or more in any country outside the UK between
present day and the start of the programme in September then please include future dates
here as well (you will need to provide a Certificate of Good Conduct from any relevant
countries prior to enrolment) :
