A nurse's therapeutic communication to their client is divided into 4 stage,namely :

1. Pre-Interaction stage
2. Orientation phase
3. Work phase
4. Termination stage

1. Pre-Interaction stage
At this stage,what a nurse mist do are :
-Verify the therspeutic program,that is : Ensuring that the client must indeed be given the action.
-Do the 6 steps washing hands using soap and running water
-Set up the tools. In that video, the tools prepared bt the nurse are:
√ NaCl 0,9% fluid
√ Sterile gauze
√ Soap liquid
√ 1 pair of clean gloves
√ 1 pair of sterile gloves
√ Oilcloth
√ Anatomic tweezers
√ Tissue scissors
√ Hypafiks
√ Wound oinment
-Put all the tools on the trolley and hold it close to the client.

B. Orientation Stage
1. Say hello and greet clients.
example: Assalamualaikum mis
2. Explain the intent and purpose of the action to be taken on the client
Example:
Alright mis, here I will do wound care, change the dressing and clean the wounds of maternal diabetes.
The purpose of this action is infection, help heal wounds, and increase client's self-esteem.
3. Asking the client's readiness before the activity is carried out.
Example:
Are you willing?
4. Give the opportunity to ask the client before taking action

Before starting this action, is there anything you want to ask?

3. The Work Phase

- Maintain the client's privacy
-Set the client's position (Adjust the client's position, so that the wound is visible and affordable to the
nurse)
-Wear a clean gloves
-Put the oilcloth under the client's wound
-Open the wound bandage, than throw it into the plastic bag
-Flush the wound slowly using NaCl 0.9% fluid
-Spray soapy liquid on the wound while occasionally cleaning the wound using a gauze
-Observation of the client's wound condition, type of wound, presence or absence of pus and wound
depth
-Get rid of rotten tissue using tissue scissors
-Replace the clean gloves with the sterile gloves
-Do wound care with gauze, which has been given NaCl solution until clean
-Apply wound oinment
-Cover the wound with sterile gauze
-Fixation of the wound with hypafiks
-Tidy the client

D. Termination stage
1. clean the equipment
2. Tell the client that the action has been completed.
For example: Alright, mis, the wound care procedure on the mother's hand has been completed. do you
feel comfortable?
3. Thank you for your cooperation
example: Thank you for your cooperation, mis.
4. Take off the gloves
5. Wash your hands
6. Notifying patient families when they may reenter
7. Say goodbye to the patient and the patient's family
