N : Would you tell me about your health condition recently?

P : Yeah nurse, i am getting better now. My eyes no longer itchy, sore, and watery.

N : Do you have any trouble with your vision?

P : Everything is back to normal

N : Sounds great, would you to eat something for your breakfast? Your body needs to recover
and have some nutrition.

P : Thank you nurse. I would like to eat risolles if any.

N : Would you like to drink juice this morning?

P : No nurse. I prefer mineral water.

DIALOG ASKING HISTORY OF PAST HEALTH AND ILLNESS

N : Have you ever had red eye disease before?

P : Yes i have nurse. It occurs unconditionally

N : Are you allergic to certain food?

P : I am allergic to nut
N : Miss, have you ever been immunized against measles?

P : Yes, i have. I got immunized when i was elementary school grade 4

DIALOG PATIENT ASSESMENT

( SIT OPPOSITE)

N : Now, i am going to check your eyes

P : Sure nurse

N : Please open your eyes. I want to check the state of your eyes ( use pen light )

P : Sure

N : I am going to examine your eyelids by pulling them down

P : ( Still opening eyes )

N : Now i am going to press your upper eyelid. Please close your eyes and please tell me if it
hurts

P : Please do it gently nurse ( Closing eyes )

N : Does it hurts?

P : No nurse

N : Now you can open your eyes again. I want to check your eyesight. Please stand up ( take
distance between nurse and patient) and follow my instruction. Please close your left eye as my
hand form a bowl. After that tell me what alphabet i ponit to now ( use snellen card ). Do the
same with the right eye.

P : Sure ( mention the alphabet designated)

N : Ok, the examine is over. Please sit back. I checked your eyes. Your eye condition now looks
really good. No visible inflammation and no water. Your eyesight is normal.

P : Sounds good. Thank you nurse.

N : I advice you to eat foods that contain lots of vitamin A like carrot, spinach, papaya, and egg.
Take a rest and drink a lot of water.
