    to formally request your permission to conduct our survey on your advisory class, the
    Grade – 8, St. Joseph Freinademetz.

    Our survey entitled “Needs Assessment for Students” aims to narrow down personal
    and academic concerns of the Junior High School, and serve as the raw data for our
    Performance Task in compliance with our Disciplines and Ideas in the Applied Social
    Sciences. With your permission, we would like to take at most 15 minutes of their time
    in answering the survey questionnaires given.
