AFFIDAVIT OF EXPLANATION

I, MYRA JACOBO VILLANUEVA, of legal age, single, Filipino and

with residence address at Barangay Minallo, Naguilian, Isabela,
after being sworn in accordance with law hereby depose and say:

1. That I am the true owner and holder of Philippine Passport

Number XX2812159 issued by Department of Foreign Affairs Consular
Office- Tuguegarao with extended validity on November 4, 2017 ;
2. That I was born on August 19, 1988 at Minallo, Naguilian,
Isabela to Sps. Aurelia Jacobo and Rudy Villanueva. Attached is
a copy of my Certificate of Live Birth under Local Civil
Registry No. 88510 of Naguilian, Isabela;
3. Before going back to the Philippines on August 26, 2017, I
applied for the renewal of my Philippine Passport at the Embassy
of the Philippines, State of Kuwait, but I could not renew the
same because another person used my name, passport number and all
my personal details except my picture in renewing her own
passport;
4. That through the advise of the Embassy of the Philippines,
State of Kuwait, I returned to the Philippines to renew my
Philippine Passport;
5. That I am executing this affidavit to attest to the truth
of the foregoing, to state that I am the true MYRA JACOBO
VILLANUEVA born on August 19, 1988 to Sps. Aurelia Jacobo and
Rudy Villanueva and I am the true holder of Philippine Passport
Number XX2812159 with extended validity on November 4, 2017, to
submit this affidavit as a requirement for my passport renewal
before the Department of Foreign Affairs-ASEANA and for whatever
legal purpose it may serve.

AFFIANT SAYS NONE.
