PLEPS: NM is a second-grade student at Otselic-Valley Central School for the 2020-2021 school year.

She
is currently receiving speech services 3x/week for 30-minute sessions in a group. NM is eager to
participate in speech and remains focused and motivated throughout each session. She has made great
strides toward many of her goals, including using -ing endings, completing sentences with a subject and
predicate, and using possessive pronouns his/her/their. Based on data collected through sessions, NM
has produced his/her/their with 100% accuracy in sentences independently, used -ing endings with
100% accuracy in sentences independently, and completed sentences using a subject and predicate with
92% accuracy in sentences independently. She will continue to work toward producing these structures
through the conversation level. Her areas of need include increasing intelligibility by producing target
sounds /r/, /l/, and /th/. Based on data collected in previous sessions, NM has produced /r/ in sentences
given cues with 100% accuracy, /l/ across word positions given a model with 90% accuracy, and /th/
across word positions, independently, with 44% accuracy. Her goal is to improve independence and
complexity through the structured conversation level.

Social: NM is eager to please adults and gets along well with her peers. She is shy at first but opens up
when she becomes more comfortable. She is motivated and engaged in the speech room and always
tries her best.

Classroom modifications: None are needed.

Goals:

1. NM will increase her speech intelligibility by producing target sounds /r/ and /l/ with 90%
accuracy in structured conversation, independently.
2. NM will increase her speech intelligibility by producing target sound /th/ with 90% accuracy in
sentences, independently.
3. NM will label regular present and past tense verbs with the appropriate verb ending, -ing or -ed,
with 90% accuracy in structured conversation, independently.
4. NM will use the appropriate subject and possessive pronouns with 90% accuracy, in structured
conversation, independently.
