1. Description of works
What does the work entails? The following are the work scope: -
 Mobilize of personnel and tools to work site
 Site work preparation
 Install prefabricated gratings in specified location
 Housekeeping
How is the work going to be completed / commissioned?
Mobilize of personnel and tools to work site
 Prior to mobilize personnel and tools to work site, the following activity shall take place,
Site Safety Supervisor shall screen all workers dossier and conduct safety induction.
PTW Applicant, Site Safety Supervisor and Site Supervisor shall visit the work site to
familiarize themselves with the area and any potential issues.
 Site Safety Supervisor and Site Supervisor shall carry out tools and equipment
inspection prior to mobilize to work site.
 PTW applicant shall apply and ensure PTW is based on work scope.
 Mobilize personnel and tools to work site.

Site work preparation


 Site Supervisor and Client inform the workers where the work shall take place
 Site Supervisor shall direct workers to install barricade and safety signage at the
location.

Installation of Grating cover :

 Site Supervisor (SS) and Client Representative to verify location where gratings are to
be installed prior to start works.
 Site Supervisor (SS) shall direct workers unload the gratings from the lorry in specified
location manually.
 Site Supervisor (SS) shall direct workers to hack drainage structure as per grating
dimension using hammer if the concrete structure is uneven.
 Site Supervisor (SS) direct the workers to Install the gratings manually.
 Repeat the steps for other locations.

Housekeeping
 Site Supervisor shall direct workers to remove barricade, safety signage and
housekeeping.
 Site Safety Supervisor, Site Supervisor and PTW Applicant ensure work site
housekeeping completed and accepted by Client representative.

4. Communicating with the workforce


How are the procedures, hazards to be communicated to the workforce?
Method Statement and JHA will be explained to the personnel by the Project Engineer / Supervisor
and Safety Supervisor during the daily toolbox meeting prior to start of work on the work scope and
the risks/hazard involved. The precaution to be taken and hazards that can be avoided, etc. This is
to ensure all personnel on site are aware of the job that they undertake and the risk involve. Minutes
of toolbox meeting will be recorded together with feedbacks and recommendations.
