Informed Consent by volunteers to participate in evaluation of “The Feasibility of

Pressing and Air-drying Water Hyacinths Using Hollow Blocks, Old Hangers, and
Other Accessible Materials in Barangay Sampad, Rizal
Purpose :
The purpose of this study is to test the feasibility of pressing and air drying
techniques on Water hyacenths in the context of the community of Sampad rizal.
This is to see if the they are efficient ways in terms of further processing water
hyacinth thus speeding up the removal of water hyacenths.

Procedures:
The researchers shall interview with questions relating towards the effectiveness of
the experiment and their personal feedback in further improving it.

Duration and location

This interview will only last 10-15 minutes to complete. This study will be
performed in the location most convenient to the participant, whether it be their
house or working place.

Participant Requirements:
They need to be within the ages of 18-59 and must have work related to Lake
Laguna.

Risks:
The risks and discomfort associated with participation in this study will be no
greater than the one encountered with talking to others.

Benefits
The information received maybe of value in terms of improving the design of the
technique/set-up which may improve the efficiency of the processing of water
hyacenths, which in turn helps with the water hyacinth problem.

Compensation and costs

Participation is completely voluntary and there will be no cost to partciapate.

Confidentiality
By participating in the study, you understand and agree that the experimenters may be
required to disclose your consent form, data and other personally identifiable information
as required by law, regulation, subpoena or court order. Otherwise, your confidentiality
will be maintained in the following manner:

Your consent form will be stored in physically in the storage files’ of researcher Bolong
and digitally in his laptop. They will not be disclosed to anyone other than the
researchers. By participating, you understand and agree that the experimental data and
information can be used for the making of further research papers on the topic. However,
your name, address, contact information and other direct personal identifiers in your
consent form will not be disclosed at any time.

Rights

Your participation is voluntary. You are free to stop your participation at any point.
Refusal to participate or withdrawal of your consent or discontinued participation in the
study will not result in any penalty or loss of benefits or rights to which you might
otherwise be entitled. The researchers may at his/her discretion remove you from the
study for any of a number of reasons.
