Note:
There are two kinds of evaluation, the formative and summative evaluation.
Formative evaluation is done at periodic points during the implementation phase to determine changes in the client's health condition or home
and environment realities. Formative evaluation guides both the nurse and the family on decisions about modifications of goals, objectives and
intervention strategies/measures, depending upon the current situation, emerging or additional health needs/problems and priorities of the
family. Summative evaluation on the other hand occurs at the end of the family-nurse relationship. It determines if the goals as specified in the
family nursing care plan are achieved as measured by the outcome criteria and evaluation standards. Based on the family's current situation
and/or remaining health needs or problems, the nurse can guide the family on making choices about termination or referral.
