JOINT-AFFIDAVIT OF TWO DISINTERESTED PERSONS

We, VIRGIE NASIAD and CRISELDA PACALDO, both of legal ages,
Filipinos, married and are residents of Hagonoy Crossing, Hagonoy, Davao del
Sur, after having been duly sworn to in accordance with law, do hereby jointly
depose and say:

That we are the same and identical person who caused for the execution
of this Affidavit;

That we personally know the person of LOURDES ABALLE, who was

born on February 11, 1935 at Hagonoy, Davao del Sur, Philippines;

That the fact of birth of LOURDES ABALLE was not registered at the
Local Civil Registrar of Davao del Sur, due to negligence and inadvertence;

That until at present the said fact of birth of the above-named person was
not yet registered in any Local Civil Registrar;

That we are not related by blood nor by affinity of the above named
person;

That we are executing this affidavit in order to attest the truth of the
foregoing facts, to apprise the proper authorities of the same and for the purpose
of registering the fact of her birth at the Local Civil Registrar of Davao del Sur,
Philippines;

Further affiants sayeth naught.

Filipinos, married to each other and are residents of Buri, Matanao, Davao del
Sur, after having been duly sworn to in accordance with law, do hereby jointly
depose and say:

That we are the same and identical person who caused for the execution
of this Affidavit;

That we personally know the person of Rubelyn A. Navarez, of legal age,

Filipino and a resident of Purok Mahayahay, Matanao, Davao del Sur,
Philippines;

That we know for a fact that she gave birth on November 29, 2009 of a
baby girl named GWENDEMLYN NAVAREZ FAMOR in Matanao, Davao del
Sur;

That the father of the said child is DEMOSTHENES B. FAMOR;

That the fact of birth of GWENDEMLYN NAVAREZ FAMOR was not

registered at the Local Civil Registrar of Davao del Sur, due to negligence and
inadvertence;

That until at present the said fact of birth of the above-named child was
not yet registered in any Local Civil Registrar;

That we are not related by blood nor by affinity of the above named
person;

That we are executing this affidavit in order to attest the truth of the
foregoing facts, to apprise the proper authorities of the same and for the purpose
of registering the fact of her birth at the Local Civil Registrar of Davao del Sur,
Philippines;

That we are the same and identical person who caused for the execution of this
Affidavit;

That we personally know the person of Mariz Rebias Delos Santos, of legal age,
Filipino and a resident of Sinaragan, Matanao, Davao del Sur, Philippines;

That we know for a fact that she gave birth on February 6, 2013 of a baby boy
named IAN RENZ DELOS SANTOS BAYAWA in the Davao del Sur Provincial Hospital,
Digos City, Davao del Sur;

That the fact of birth of IAN RENZ DELOS SANTOS BAYAWA was not
registered at the Local Civil Registrar of Davao del Sur, due to negligence and
inadvertence;

That until at present the said fact of birth of the above-named child was not yet
registered in any Local Civil Registrar;

That we are not related by blood nor by affinity of the above named person;

That we are executing this affidavit in order to attest the truth of the foregoing
