1. Introduction
The Project, Pakistan Kidney and Liver Institute and Research Center, Lahore (PKLI)
will be a state of the art center of excellence in clinical care, teaching and research in
specialized areas of kidney, bladder and liver diseases including kidney and liver
failure, treated with transplantation. The project will include 800 beds Teaching
Hospital, a mosque, state of the art school of nursing and paramedics that will also
have accommodation for 1000 students and their faculty, research lab and separate
accommodation towers for nurses, paramedics, doctors and patients along with a
commercial area and additional area for supporting allied production units to make
revenue generation for assisting sustainability of PKLI. This is a charity project where
patients will not be charged.
2. GENERAL:
The Employer Pakistan Kidney & Liver Institute and Research Centre (Trust), Lahore
issues this Pre-qualification Document to applicants interested in bidding for works /
services as described in Scope of Works. We are specifically looking for Local and
International companies which can finish a fast track project of 800 beds, built on
International standards in limited time with highest quality and within budget.
a) Source of Funds:
The Employer Pakistan Kidney & Liver Institute (PKLI) And Research Centre
(Trust), Lahore has their own funds.
b) Scope of Work:
The scope of Works to be performed shall consist of Civil, HVAC, Plumbing and
Electrical Works and for execution of Works deployment of all necessary labour(s),
Tools & Machinery and supply, Installation, Testing & Commissioning and
Operation & Maintenance, sale service warranty of supplied equipment and necessary
spare-parts of approved Plant & Equipment including specified materials for
accomplishing and maintenance of the works covered under tender for Construction of
Hospital with Energy Centre at Badian Road, Lahore. The scope of Works with
tentative covered area shall be in accordance with the items listed below:

3. ELIGIBLE BIDDERS
a) Eligible Bidders
General Contractor(s) / JV / Association shall be Eligible subject to
compliance of following conditions as follow;
i. The Applicant(s) are not blacklisted with any Government or
Semi Government Organization (Provide a statement on a
Stamp paper of minimum Rs. 50 pertaining to non –
blacklisting with Government or Semi Government
Organizations).
ii. General Contractor(s) / JV / Association are registered with
FBR with a valid NTN number (Copy of NTN Certificate shall
be provided).
iii. General Contractor(s) / (Individual / JV / Registered
Partnership Firms) JV / Association having Valid License from
PEC in main category C–A and relevant sub categories (in
respect of their applied scope of work) or / and their associates,
sub-contractors / sub-associate(s)/JV members, having valid
PEC license for relevant scope of work, (if applicable).
iv. PQ # 01 shall be filled to provide General Information relating
to all participated bidders.
v. PQ # 16 shall also be provided to give authentication to the
information provided in the Questionnaires.
b) Qualification Criteria:
1. Pre-qualification will be based on applicant‟s meeting all the following
criteria regarding their general and particular construction experience,
financial soundness, personnel and other relevant information as
demonstrated by the applicant‟s responses in the Pre-qualification
Questionnaire Forms attached to the Letter of Application. Additional
requirements for joint ventures /Association given in Section 2, subject
to Eligible Applicant(s) in provision with sub clause 2.1.
In general:
An Applicant may be a private, public, government or proprietorship
owned legal entity, or any combination of them in the form of Joint
Ventures (JV), association(s), with the formal agreement.
2. In the case of a JV/ Association, shall be as per clause 6;
i. All parties shall be jointly and severally liable, and
ii. There shall be no limit on the number of members.
3. The eligibility criteria listed in this document shall apply to all the
Applicant, including the parties constituting the Applicant, i.e. its
proposed members/partners, associates, JV partner(s), sub-contractors

4. EVALUATION CRITERIA
Applicant(s) meeting the minimum requirements mentioned in Para 2.2 besides other
factors shall be considered for pre-qualification. No compromise shall be made on
minimum requirements of 50% score in each category and an overall minimum of
70% marks shall be required to prequalify in the aforesaid scope of work.
5. LETTER OF APPLICATION

6. PREQUALIFICATION DOCUMENT
a) Clarification of Pre-Qualification Document
A prospective Applicant requiring any clarification of the Prequalification
Document shall contact the Employer in writing at the Employer‟s address
indicated in the PQ, Instruction to Applicants. The Employer will respond in
writing to any request for clarification provided that such request is received
no later than five (5) days prior to the deadline for submission of applications.
The Employer shall forward copies of its response to all applicants who have
acquired the prequalification document directly from the Employer including a
description of the inquiry but without identifying its source. The Employer
reserves the right to amend the prequalification document as a result of a
clarification as per departmental procedures and communicate to all applicants
in writing.
d) Language of Application
The application as well as all correspondence and documents relating to the
prequalification exchanged by the Applicant and the Employer, shall be

prepare a record of the opening of applications that shall include, as a
minimum, the name, signature of the representative (if they choose to attend).

The Employer reserves the right to waive minor deviations in the qualification
criteria if they do not materially affect the capability of an applicant to
perform the contract.
7. Employer Right
Bidders / Applicants wishing to pre-qualify shall complete the Pre-
qualification Questionnaires / Qualification Criteria and other relevant details
solicited through pre-qualification documents including the qualification
criteria in duplicate and return them to the address given in a sealed envelope.
b) Member Limitation
There shall be no limit on the number of members in a JV/Association; PQ #
13 shall be filled to affirm all the members of the JV / Association.
9. INSTRUCTIONS TO APPLICANTS
a) Submission& Opening of Pre-qualification Application
Application for Prequalification (One original and One Copy) shall be
received in sealed envelopes (properly mentioned on the Envelop in clear
visible writing) to be delivered by hand or through registered mail on or
before Nov. 20, 2015 at 1700 Hours PST.
c) Lack of Information
Failure of an applicant to provide comprehensive and accurate information
that is essential for the Employer‟s evaluation of the applicant‟s qualification
or to provide timely clarification or substantiation of the information supplies
may result in disqualification of the applicant and same shall be
communicated in provisions with PPRA Rules.
d) Material Changes
Applicants and those subsequently pre-qualified or conditionally pre-qualified
shall inform the Employer of any material change in information that might
affect their qualification status. Applicants shall be required to update key pre-
qualification information at the time of bidding. Prior to award of contract, the
lowest evaluated bidder will be required to confirm its continued qualified
status in a post qualification review process.
a) Conditional Pre-qualification
An applicant may be “conditionally pre-qualified” that is, qualified subject to
certain specified non-material deficiencies in the pre-qualification
requirements being met by the applicant to the satisfaction of the Employer
before submission of bid.
