Preparing Medication Procedure

1. Receive and check the validity of the prescription the patient has given.The

prescription should contain the prescriber details, patient details, and the medication

details.

2. Check if their are any prescription errors present.

3. Ensure that the request meets the legal requirements for the medication to be

dispensed.

4. Ask the patient about their condition or contact their physician in order to

determine why such medication is prescribed.

5. Check for patient-specific factors such as their age, weight, if they are allergic to

the prescribed medication, and other health conditions the patient may be

experiencing.

6. Check if the dosage of the medication is safe and appropriate for the age and

weight of the pediatric patient.

7. Select the appropriate drug that the patient has been prescribed with. Check the

brand, strength, form, and quantity of medication to be given.

8. Prepare the medication for compounding.

Dispensing Medication Procedure

1. Ensure the quality of the drug and the number of the paper tabs to be dispensed.

2. Create and attach a label to the medication to be dispensed. The label should

contain the name of the medication, drug dose, directions for use, the quantity of the

medication to be dispensed, and the initials of the pharmacist who dispensed the

medication.
3. Record the date when the medication is dispensed in order to keep track on the date

and to be able to verify when the medication should be refilled.

4. Consult the patient about the medication to be dispensed. The topic of the

consultation should include the proper use of the medication, the proper storage for

the medication, when should the medication be refilled, and the possible side effects

the patient may experience upon taking the medication.

5. After consultation, ask the patient to repeat all of the instructions given about the

medication in order to ensure that they full understand what was discussed. Repeat the

consultation if necessary.

6. After ensuring that the patient understood the instructions given, Dispense the

medication.
