Why is there a need to revise the hospital formulary?

A formulary system is a multidisciplinary process that is based on research and evidence

to guide organizations on providing medications to patients. The formulary system is managed
by the P&T committee, which consists of a group of pharmacists, nurses, doctors, administrators,
and quality management personnel. The goal of the formulary system is to manage medications
and drug products to offer safe, cost-effective, and beneficial outcomes for patients.

The many parts of a formulary system include: drug formulary (a list of preferred drugs
and related products that the organization uses); Policies and procedures for acquiring,
dispensing, administering, and utilizing medications; prescribing guidelines; Clinical information
on uses of medications; and Policies for when to use non-formulary medications.

Hospital formulary is an important written document which contains the list of

preparations including important information’s which reflects the current clinical views of
medical staff. It is the painstaking effort of any hospital so as to guide medical and paramedical
staff. Moreover, it is very educative and useful to the members of “Health Care Team” hence
copies of formulary should be placed at each patient care unit, including clinics, out-patient care
areas and emergency room.

Now why is it important to revise it? Formulary systems provide the procedure to select
the appropriate drugs for different diseases, conditions, and patients. Now there will be instances
like generally for addition, deletion, change in the drug products, removal of drug from the
market, change in the hospital policies and procedures therefore the formulary need revision
annually. Furthermore, in the present scenario, each and every hospital should adopt a formulary
and of course revise it for good health practice and good inventory control measure that provide
a wide choice for the physician to fulfill the needs of patients.

Since I don’t have enough knowledge, I have gathered my thoughts through further research on
drug formulary systems so as to know more about hospital formulary systems. The following are
