As we then continue, we would like to assist you in drawing your family into this journey of faith.
In your initiative and pacing, plan for activities that would bring your family closer with one
another and most especially closer to God. Use the format below to plot your activities. These
activities are to be carried monthly. For checking and monitoring purposes, you are to submit them
following a designated scheme. These will be compiled and supported by documentations (photos)
and will serve as your final requirement for your respective Rel Ed subject in the current semester.

GENERAL GUIDELINES and SCHEDULE OF SUBMISSION

1. Accomplish the MONTHLY ACTIVITY PLAN and submit as a draft to your Rel Ed. teacher.
a. Take note that the activities that you shall indicate in the activity plan are those that you
can commit to undertake with your family be it a daily, weekly or once a month basis.
b. In the conduct of your activity, make sure that you are also taking some
documentations [pictures] which you will be needing to submit as a report for your
output. A simple or short description of the actual activities may also be of help.
c. Monthly outputs shall bear a Reflection for your Monthly activity summarizing your
journey of faith.
d. All outputs shall be posted in your MST TEAMS Class Notebook tabbed as: FAMILY
EVANGELIZATION.

Submission of DRAFT is every 1st Week of the Month.

This is to enable your instructor to check on them and monitor your activities.

Submission of MONTHLY OUTPUT is on the First week of the Next Month

which will be inclusive of your draft for the next month also. This will be done in
succession.

2. For the teacher’s reference, a document that bears the evidences and learning insights of you
Family Evangelization Activity will be submitted during finals as a major requirement. It shall
follow the format:

1. Title Page. This page bears the title of the activity. At the bottom is the name of the student
submitting the paper [family name first, course, year and section]. See sample below.

2. Introduction. Here stated are your vision, ideas and expectations in going through the activity.
3. Profile of the respondent. A family picture and description of the members of the family.
4. Monthly Activity Plans. Simply collate the activity plans you were able to submit. [using the same
format]
5. Documentation. These are still pictures properly captioned that testifies the extent of your
participation and accomplishments.
6. Personal Reflection. Generally, what is the impact of the activity? What was your participation
and realizations? [Lessons learned and experiences and commitments.]

Note: All outputs that the final document will contain should be set in a letter size paper [8.5 x 11]. In case we
needed it for printing then it is easier to produce the document for whatever purposes it may serve.
