Look over info, create your own, discuss with team, come up with one and do a presentation (SBAR and
the other one)
Youtube vid: SBAR – POOR: Important to start who you are, where you calling from, the patient’s name,
also remember attending patient.
Discuss with doctor, “which side are you operating on? Let the doc respond what’s the right side. Don’t
come off as confrontational. Let him educate you.
ISBAR
Introduce
Situation: the problem. State your name and where you are calling from.
State the: admission diagnosis, date of admission, medical history. A brief synopsis of the treatment to
date.
Pertinent and objective and subjective information. Mental status. Most recent vitals, skin color, RR and
quality, BP, Pulse rhythm, quality. Neuro changes, Pain.
Request: what you want. Transfer/admit the patient? Change treatment? Come to see the patient?
When? Talk to the family and patient about…? Ask for a consulting physician to see the patient?
Suggested diagnostic evaluation (Labs, Rad, other); Change in treatment, consults, ask “ if the patient
does not improve, when would you want to be called again?”
Be confident and composed, maintain eye contact, use clear, concise speech, speak firmly and
positively, be non-apologetic, take initiative and guide the situations, give the same message verbally
and nonverbally, speak genuinely, without sarcasm. Don’t say I am sorry to call you late at night (it’s
their job)
Make sure to have her attention. Trigger word “my plan is”
“Hello, this is PA< Jenny from OB at Pace University Medical Center. I am calling to let you know that I
am admitting Mrs. Windlow, a patient of your partner.”
A is better, SP
B is better. Don’t say VS are stable (state it)
